Draft Community Performance Indicators
Medicaid Integrated Managed Care is expanding into many states across the country. It is important that HMO's have accountability measures so advocates can monitor what they are doing. These accountability measures should be in the contract linked to reimbursement so the HMO's have an economic incentive to perform in a way that benefits the people receiving services.
ADAPT of Texas has drafted what we are calling Community Integration Performance Indicators.
Community Integration Performance Indicators:
- Number of people out of nursing facilities/institutions;
- Number of people going into nursing facilities/institutions;
- Number of people getting face to face service coordination;
- Number of people getting phone service coordination;
- Number of people offered consumer directed services;
- Number of people selecting consumer directed services;
- Number of people living in their own home or apartment;
- Number of people living in assisted living;
- Number of people in adult foster care;
- Number of people living in group homes;
- Availability/use of architectural barrier modifications;
- Length of time receiving services;
- Lenth of time keeping an attendant;
- System of back up for attendants;
- Pay wages $7.25 to $8.00;
- Pay wages $8.00 to $9.00;
- Pay wages above $9.00;
- Access to durable medical equipment
- Access to Assistive Technology such as communication devices
- Nurse delegation of health maintenance task to unlicensed Direct Care Attendants
- Advisory Committee made up of at least 50% of people using the services and supports
